How it works
Fluticasone furoate is a synthetic trifluorinated corticosteroid with potent anti-inflammatory activity. The precise mechanism through which fluticasone furoate affects rhinitis symptoms is not known. Corticosteroids have been shown to have a wide range of actions on multiple cell types (e.g. mast cells, eosinophils, neutrophils, macrophages, and lymphocytes) and mediators (e.g. histamine, eicosanoids, leukotrienes, and cytokines) involved in inflammation. These anti-inflammatory actions of corticosteroids may contribute to their efficacy in rhinitis.

Side Effects
• Nosebleeds (generally minor), particularly if you use this nasal spray for more than 6 weeks continuously • Nasal ulceration – which may cause irritation or discomfort in your nose. You may also get streaks of blood when you blow your nose. • Headache. • Shortness of breath • Pain, burning, irritation, soreness or dryness in the inside of the nose • Small holes (perforations) in the ridge inside the nose that separates the nostrils .
Drug Interactions
• Steroid tablets or injected steroids • Steroid creams • Medicines for asthma • Ritonavir or cobicistat, used to treat HIV • Ketoconazole, used to treat fungal infections .
Indication
It is indicated for the treatment of the symptoms of seasonal and perennial allergic rhinitis in patients 2 years of age and older .
When not to Use
Do not use If you are allergic to fluticasone furoate or any of the other ingredients of this medicine .
Kartico Nasal Precautions
Precaution
Visual disturbance may be reported with systemic and topical corticosteroid use. If a patient presents with symptoms such as blurred vision or other visual disturbances, the patient should be considered for referral to an ophthalmologist for evaluation of possible causes which may include cataract, glaucoma or rare diseases such as central serous chorioretinopathy (CSCR) which have been reported after use of systemic and topical corticosteroids.
Kartico Nasal Warnings
Warning 1
Monitor patients periodically for signs of adverse effects on the nasal mucosa. Avoid use in patients with recent nasal ulcers, nasal surgery, or nasal trauma, because of the inhibitory effect of corticosteroids on wound healing.
Warning 2
It is recommended that the growth of children receiving prolonged treatment with nasal corticosteroids is regularly monitored. If growth is slowed, therapy should be reviewed with the aim of reducing the dose of nasal corticosteroid if possible, to the lowest dose at which effective control of symptoms is maintained. In addition, consideration should be given to referring the patient to a paediatric specialist .
Warning 3
Concomitant administration with ritonavir is not recommended because of the risk of increased systemic exposure of fluticasone furoate .
